About our project

Background
Osteoarthritis (OA) is one of the top ten diseases at population level, with hip and knee joints mostly affected, and most of the people with complaints will be seen in primary care. Little is known about the natural course of hip OA and the factors that influence the prognosis of hip OA. Cohort hip and cohort knee (CHECK) is a long term (10 year) follow-up study in people with early hip and knee complaints (first attenders to primary care).

Aim
We aim to explore trajectories of pain in people with hip complaints and hip OA. Furthermore, we aim to explore factors that are related to these pain trajectories, and factors that predict joint replacement in the future, to give general practitioners more knowledge about the prognosis of hip complaints.

Methods
The CHECK study is a prospective 10-year follow-up cohort in the Netherlands of 1002 participants with hip and/or knee complaints. Participants were included at or within 6 months of their first visit to the general practitioner for hip or knee complaints. Patients with mild symptoms were followed at years 0, 2, 5, 8 and 10, patients with more serious symptoms were followed yearly. Examination included extensive questionnaires and physical exam for clinical data, and radiographic and biochemical variables. Different statistical analysis will be used to define the pain trajectories and prediction of pain trajectories.

Our research focus

Primary outcomes
The primary outcome is clinical OA, based on the ACR (American College of Rheumatology) criteria for OA during follow-up.

Secondary outcomes
Secondary outcomes are structural OA, based on x-rays, joint replacement during follow-up, and pain trajectories.
