Change your personal details online, using My Erasmus MC
You can change the following details yourself in Mijn Erasmus MC:
- Your personal details, telephone number or email address.
- The name and contact details of your GP.
- The name and contact details of your pharmacy.
You can also use the contact form to let us know about any changes.
Please do not include any confidential medical information in the contact form.
Change your personal details in the hospital
You can change your personal information in the hospital at the registration desk, at the registration kiosks or sign-in kiosks. You must do this before your appointment at the hospital. Do not forget to bring your identity card.
At the registration desk, a staff member will help you change your information. At the registration kiosks and sign-in kiosks, you can change your data yourself.
