Please let us know as soon as possible if there is any change in your child’s personal details, e.g. the home address or telephone number, or the details of his or her GP, pharmacy or health insurance policy.

You can go to the registration desk
You can also go to the registration desk before your appointment to change your child’s details. The registration desk is located in the main hall of the Erasmus MC Sophia Children’s Hospital.On-line, using My Erasmus MC
You can change the following details yourself on Mijn Erasmus MC
- your child’s personal details, telephone number and email address;
- the name and contact details of your child’s GP;
- the name and contact details of your child’s pharmacy.
Please note that parents and guardians of children under the age of 12 are entitled to access their medical records. In order to do so, you should log-in using your child’s DigiD code. You can find instructions on how to use the DigiD code under ''Hoe werkt DigiD?” (How does DigiD work?).
Check your child’s details once a year
For your child’s comfort and care in hospital (and also to ensure that we can invoice your healthcare costs correctly), our record of their personal details must be kept up to date. For this reason, you are kindly requested to check your child’s details once a year, preferably before your hospital appointment. Please bring your appointment card and your hospital pass with you.
