My Erasmus MC

My Erasmus MC is our online patient portal where you can access your medical records,  test results and letters from physicians. It also contains a list of all your appointments. You can login to My Erasmus MC using your DigiD. Do you not have a DigiD yet? You can request one at www.digid.nl.

Requesting a copy of your medical records

You may want to have access to medical records that are not included in My Erasmus MC. If so, you can contact us to receive a copy of these medical records. It is usually not necessary to request a full copy of your medical records. Often a few parts are sufficient. You should always submit your request in writing, you cannot complete the form online. Please use the ‘application form for medical information’.

How do you make a request? 

Receiving a copy of the medical records

On the application form, you can indicate that you would like a copy of a radiological examination with your medical records.

If you only want a copy of a radiological examination, you can also visit the Radiology & Nuclear Medicine desk in the Nd building on the 5th floor. Please bring a valid ID. Without a valid ID, you will not be given a copy of your examination data. The copy of your X-ray, MRI or CT scan will be burned onto a CD-ROM.

Would you like a copy of an examination from before the year of 2005? Then first call the Radiology & Nuclear Medicine Department on telephone number (010) 704 20 06.

If applicable, please also read the information under copy of medical records of children, copy of medical records of IVF/ICSI treatment, copy of medical records of deceased patient or copy of medical records requested by third party.

The rights to receive a copy of children's medical records, including radiological examinations, are different for 3 age groups:

• Child is under the age of 12
  Parents and legal representatives have the right of access to the medical records of their children under the age of 12. They may request a copy of the medical records.
• Child is aged between 12 and 16
  Parents and legal representatives have the right of access to the medical records of their children aged 12 to 16. They may request a copy of the medical records, but the child may object to this. It is therefore necessary that the child co-signs the application form.
• Child is aged 16 and over
  The rules for adults apply to children aged 16 and over. They can make their own application.
  If the treating physician feels that providing a copy is incompatible with the care of a good caregiver, the provision of a copy may be denied.

In principle, a noncustodial parent is only entitled to general, factual and important information about their child's treatment. The treating physician follows the law to determine whether he may provide the requested medical information.

When requesting a copy of (part of) the medical record of a child under the age of 16, both custodial parents must sign the application and both must enclose a copy of their valid ID.

Do you want to request a copy of your medical records regarding a fertility treatment such as IVF or ICSI? If so, it is necessary for both partners to sign the application form. You can place a second signature on the form under ‘partner’s signature’. A copy of a valid identity document is also required from both partners. You can only request a full copy of the file.

There are strict requirements when requesting a copy of the medical records of a deceased patient. If you would like to request a copy of the medical records of a deceased person, you must submit the form 'explanatory notes on requests by third parties' in addition to the  application form. On this form, you describe the reason for your request. Your request will be evaluated based on the given explanation.

A request for a copy of a medical record of an incapacitated patient can be made by the legal representative, such as curator or mentor. If the attending physician considers providing a copy incompatible with the care of a good caregiver, the provision of a copy may be refused. By the way, this applies to all requests, not just when a copy is requested by a third party.
