If you are visiting Erasmus MC for the first time, it is essential that you register as a patient at least 30 minutes before your appointment. You can complete your registration at the registration desk or the self-service registration kiosk.

It is important that you register as a patient at Erasmus MC before coming to your first appointment. You can register at the registration desks or at the self-service registration kiosk in the hospital's registration plaza. Once you have completed your registration, you will receive a day ticket that displays the time and location of your appointment. You can find additional information on this page.

If you have previously registered with us, you may retrieve a day ticket from one of our self-check-in kiosks.

Check your personal information in My Erasmus MC

We have already received some of your personal information through your referral. To ensure that your personal information is accurate, we encourage you to check your data in My Erasmus MC , our online patient portal.

It is important that your personal information is correct. Please review your personal data and fill in any missing information. This will save you time during your hospital visit.

Registration at the registration desk

Please arrive at the Registration Plaza 30 minutes before your appointment to complete your registration. You can do this at one of the registration desks or self-service registration kiosks.

At the registration desks, a staff member will assist you with your registration and take a photo of you for your records.

At the self-service registration kiosks, you can complete the registration process yourself. The kiosks will automatically scan the photo from your identification card.

It is essential that you visit us to complete your registration because we are legally obligated to verify your identity. Therefore, please remember to bring your identification card with you to the hospital.

Once your identity has been verified, your registration will be complete, and you can proceed to your first appointment.

Registration desk opening hours: Monday to Friday, 7:00am-5:00pm.
Self-service registration kiosk opening hours: Monday to Friday, 8:00am-3:00pm.

Follow the signs for 'Registration' or ask one of our hostesses or hosts for directions. You can also find the locations of the registration desks and self-service registration kiosks on this map of the Erasmus MC. The registration locations are recognizable by the large white areas with 'registration desk' in them.

After your registration, you will receive a day ticket with the time and location of your appointment. Our hostesses and hosts will be happy to guide you.

What do I need to register?

You will need the following for your registration:

Valid identification (passport, identity card, residence permit or driver's license).
Name and address of your general practitioner.
Name and address of your pharmacy.

When you register at Erasmus MC, we ask for your e-mail address. We will use your e-mail address for communication about your treatment.

Patient satisfaction survey

We use your email address to communicate about your treatment, but also to ask how you experienced the care. What went well and what could be improved according to you? Your opinion is indispensable for improving our care.

If you would like to participate, we will occasionally send you an email with a questionnaire. We process your answers anonymously.

Don't want to receive questionnaires?

If you do not want us to contact you for questionnaires, you can indicate this during your registration. We will then only use your email address for communication about your treatment. We always respect your choice.

Follow-up appointment

For a follow-up appointment, you do not need to register again. You do need to get a day ticket from one of the self-check-in kiosks at each subsequent visit before your appointment. You will need a valid form of identification for this.

Do you only have an extract from the personal records database (BRP)? Then please report to the registration desk. You will receive the day ticket there. This applies to every follow-up appointment until you have a valid form of identification.

Changing personal details

It is important that we have the correct personal information about you. If your personal details change, such as after a move, it is important that we are informed.

You can change your details online yourself via 'My Erasmus MC' or submit them via the contact form. You can also have your details changed at the registration desk in the hospital or do this yourself at the registration or check-in kiosks.
