What we do
About our project
Requesting and documenting personal information is not structurally embedded in healthcare. As a result, healthcare providers do not know patients' preferences and discussions about them do not take place.
With our project we contribute to the application and exchange of advanced care planning in (hemato)oncology patients by different healthcare providers in the 1st, 2nd and 3rd line within the oncology network Concord. So that personal preferences and treatment agreements made result in a customized treatment plan.
We will implement and secure a regional care standard for advanced care planning through which structural digital exchange of the wishes and preferences of (hemato)oncology patients takes place between all involved care providers in the Concord oncology network. So that patients can participate more actively and the care better matches their preferences. In addition, we improve transmural communication between healthcare providers and reduce the burden of registration.
